The majority of Financial Aid documents are submitted electronically via your my91ÖÆƬ³§Pro account. Follow these steps to determine if you need to submit additional documentation:

  1. Login to my91ÖÆƬ³§Pro
  2. Select Financial Aid on the Student Links Card
  3. Select the appropriate Award Year
  4. Your requirements are displayed on the Home Tab

If you have documents that need to be submitted they will be listed as ‘Unsatisfied Requirements’. Electronic forms are completed in Dynamic Forms.

If you can, you should appear in person with a valid government-issued ID to submit this form. If you cannot appear in person, you need to print this form, sign in the presence of a notary, and send the original signed document to the financial aid office. You cannot email this document.
