The majority of Financial Aid documents are submitted electronically via your my91制片厂Pro account. Follow these steps to determine if you need to submit additional documentation:
- Login to my91制片厂Pro
- Select Financial Aid on the Student Links Card
- Select the appropriate Award Year
- Your requirements are displayed on the Home Tab
If you have documents that need to be submitted they will be listed as 鈥楿nsatisfied Requirements鈥�. Electronic forms are completed in Dynamic Forms.

If you can, you should appear in person with a valid government-issued ID to submit this form. If you cannot appear in person, you need to print this form, sign in the presence of a notary, and send the original signed document to the financial aid office. You cannot email this document. |
